2017 Spring Field Day Registration Form
Please use this form to register your family to participate in Visions Annual Spring Field Days. A fully completed form will enable us to properly plan and staff each event. ALL participating children and adults MUST wear closed toe sports shoes.

At the event, students will compete in a variety of field games supervised by the Visions staff. Hydration and snacks will be provided for the participating students.

During the activities, parents/guardians will have an opportunity to network with  CTs, administrators and other Visions home school families. A family barbecue will be offered from 12:00 - 12:30.

This is a "no fee" event and open to all K-12 Home School students and their immediate family members.

How many non-enrolled SIBLINGS will be attending? *
Do not list infants or children that will not be participating in games or the barbecue. Siblings who are old enough to follow directions may join in the games.

Adult is defined as anyone named on the student's Master Agreement or living in the household.
A hot dog barbecue will be served. Please indicate your family's choice *

You may choose any of the field days. All location events times are 9:30 am -12:30 pm (Barbecue from 12:00 - 12:30)
