This form helps us to understand how your personal boundaries interact with some of the need for heightened and charged material that may arise in a production. This form is not a contract; neither Every Story nor any of its employees or contractors will not consider any information disclosed in this form a commitment by the actor. Instead, this form is the beginning of a discussion to ensure that our production is mentally, emotionally, and physically safe for every participant.
