 House Rules:

Must be completed for participants age 18 and older, or 13 to 17 by a parent or legal guardian (Print up to three names/birthdates below of children of the SAME parent or legal guardian):

We understand with life's highs and lows, sometimes you want to break the rules. Well you have come to the right place! With that being said, we here at SHATTER ZONE want you to be safe while having a great time! So for YOUR OWN SAFETY, we have some house rules each and every participant must agree too, sign, and follow in order to participate before entering the rooms.

Participants must either be over the age of 18 or aged 13+ with a parent/guardian who will be staying on the premises during the session.

All Participants must have a signed House Rules Participation form and waiver on file before participating.

Must wear all protective equipment at all times when participating in a Shatter Zone activity.

Closed toed shoes are required! No exceptions. Absolutely no sandals, flip flops, crocs, ladies flats, heels, etc.

All weapons/tools used will be provided by the facility. You may bring in your own smashables, but not your own weapons.

All outside items to be smashed will be reviewed by our staff first. Some items such as batteries, CRT TV's or halogen bulbs for example are not allowed. You are allowed to bring up to 10 additional glass items for free as long as they are clean. 10 total per group, not per person!

You may use your phone for photos, videos or live streaming :) .

If you or others in your party appear to be intoxicated you will not be allowed to participate.

Only 2 people are allowed in a room at a time. However, others can watch from outside the room, and can tag-team in and out as needed.

Smashing/hitting of the room walls is not allowed.

Hitting yourself or others is not allowed.

Only smashing of the breakable items provided to you or that you brought in, are allowed.

 The rooms will be monitored by video. If a participant is caught violating the house rules or waiver, the session will end immediately. The participant WILL BE asked to vacate the premises or banned from entering SHATTER ZONE in the future.

I have had sufficient opportunity to read this entire document. I understand this Agreement, and I voluntarily agree to be bound by its terms. I further certify that I am the parent or legal guardian of the child(ren) listed above on this Agreement or that I have been granted power of attorney to sign this Agreement on behalf of the parent or legal guardian of the child(ren) listed above.
