NSC Game Change/Field Request Form
This form should be used by NSC Coaches to postpone or reschedule a game.

For any game postponements or reschedules, you must contact your opposing coaches and ensure both teams are aware and in alignment on any changes prior to submitting this form.

Instructions
  1. Before requesting a field for a game reschedule, CLICK HERE TO VIEW THE MASTER SCHEDULE to get an idea of what is likely available. 
  2. Ensure your opposing team’s coaches are in alignment with the proposed new date/time 
  3. Submit the form
  4. NSC Admin will confirm your game via email, and update in BAYS or GotSport
