I, (insert name below), hereby give permission to Threads Fashion Show to photograph, videograph, utilize my name, and utilize any photos taken behind the scenes of Threads Fashion Show, for Threads Fashion Show and during the production that may include me, for any and all public relation purposes as well as promotional purposes. *
If I am a model scheduled to participate for the 2026 production, I agree to attend any and all mandatory and scheduled model practices leading up to the show date, or give prior notice of my absence, and understand that failure to attend or give notice will result in being an inactive model for the show. I agree to attend Judging Day for the duration of my designer's assigned time slot or longer if I am needed. I understand my specified date and time for the sequence I am participating in as well as commit to this specified date and time to be a part of the production as a model. I understand that auditioning to model for Threads Fashion Show does not guarantee that I am selected by a designer or assigned to model for a collection. I understand that I may be delegated as a backup model to stand in during absences, and I consent to this position. (Type name below to confirm agreement to the above terms). *
