I, the legally competent adult who is a parent or guardian of the above-named child, hereby give my approval to his/her participation in any or all of the activities of the Govans Elementary Dragons After Dismissal After School Program. I understand that a guardian must be at home. While Dragons After Dismissal After School staff will be seeking to provide engagement, the responsibility of guardianship lies solely with the legal guardian. I understand that participation in these events is at my and my child's own risk. I voluntarily and intentionally release Dragons After Dismissal After School Program, its directors, employees, organizers, and sponsors from all liability loss, damage, injury, or death, including any claims based on ordinary negligence, actions or inaction connected in any way with such participation. I also agree to cover the Dragons After Dismissal After School Program for claims made by or for the participant or claims arising from any relation with the participant or the participant’s estate. Virtual Program Agreement: Please take the time to review these guidelines with your child. Parents/Guardians should supervise their children online, either directly or indirectly, subject to each child's age, maturity and parental discretion. -For all two way classes (ZOOM) I will have the device in an open area and ensure everyone is appropriately dressed when interacting online. I understand that for all two way classes only allowed registered participants and approved staff may participate. -Online conversations and actions should always be appropriate. -Privacy is very important. I will not share personal information over the internet such as phone number, address, age, passwords ect. -I understand that Cyberbullying will not be tolerated. Cyberbullying includes sending, posting, or sharing negative, harmful, false, or mean content about someone else causing embarrassment or humiliation. Photo Release: Pictures and videos will be used by the Baltimore Curriculum Project (BCP), Govans Elementary School, LETS Go Boys and Girls, Bridges Maestro, and Be a Chef for a Day websites, annual reports, in email newsletters, brochures and other web and print publications. By participating in the Dragons After School Program students who are present during such virtual events may be photographed/videoed by staff of organizations mentioned above. If there are reasons that students cannot have photographs or videos taken of them, legal documentation must be given to program director. *
Things program director (Ms. Devon) should know. ex.) students sharing a device, upcoming issues with full attendance, poor Wi-Fi connection, ect. *If things come up later please let us know, we are really depending on parents and students to tell us what needs to happen especially while virtual.
