1. If selected as a member of the North Middle School Dance Team, I understand that attendance to practices and performances are mandatory. I will schedule appointments and any other activities around practice and performance times to the best of my ability. Any potential conflicts need to be brought to the attention of the coaches PRIOR to appointment to the team. *
2. If selected I will follow the “athletic code” established by Menomonee Falls North Middle School for student athletes. I commit to representing myself, school and my team in a manner, which is beyond reproach at all times. Violations to this athletic code will not be tolerated. *
3. If selected, I commit to maintaining my grades and attending school regularly. I am aware that if I do not attend school, I cannot practice or perform.

4. If selected, I and/or my parents are responsible for all expenses. I understand that this sport is expensive and will talk to the coaches if a payment plan is needed. *
5. I am trying out for this team because I love to dance! I understand that this is a big time commitment and I will not quit or have a change of heart mid-season. I also understand that the North Middle School Dance Team continues to improve when its members return season after season throughout their Middle School career.

6. As a parent, if my dancer is selected, I will commit to helping them be the most successful member of the North Middle School Dance Team they can be. I will support the coaches in helping my dancer to follow all rules and expectations. I realize that my dancer will be expected to attend all practices and performances

7. As a parent I agree to assist with fundraising and team events as needed. I understand that parental involvement is a huge factor in the success of the North Middle School Dance Team program
