Dixon's Dance and Fitness Studio Waiver 2026-2027

By clicking the box, The undersigned releases Heather Dixon's Dance and Fitness Studio from any and all liability. The Signature on this release form indicates that I have read and understood all information, policies associated with Dixon's Dance and Fitness Studio. The studio may use all photography and video for promotional purposes.

Year End Performance Contract Season 19

By clicking the box, I give permission for my child to participate in Dixon's Dance and Fitness Studio's Year End Performance. I understand the commitment and will have my child attend all classes and rehearsals that are mandatory for my child's safety and to allow them the best possible experience.

Mandatory Dress Rehearsal Friday May 21st @ 5:30-9:00 pm

Mandatory "Entrances & Exits and Curtain Call" Rehearsal for ALL dancers. Saturday, May 7th @ 4:15 pm with volunteers meeting to follow.

One person from each Family are asked to Volunteer. (there are options that allow you to see the performance)
