2025 Convocation and Scholarship Information
This is very important for ALL SENIORS - PLEASE FILL OUT FORM COMPLETELY and talk to your counselor if you have any questions!  Do not wait on this! 

In order for your information to be in the program for Senior Convocation, this form needs to be submitted by Wednesday, MAY 1st, no later than 2:00PM!  

Please share ALL scholarships you are offered, even if you do not accept the scholarship or are attending a different school.  You may submit this form multiple times as scholarships are received.  

If you have been accepted into a branch of the military, please see the last box on this form.

* Concordia University GPA Scholarship - $10,000 - renewable - total $40,000
* UWM Music Scholarship - $2,000 - nonrenewable - total - $2,000
