ECA Summer Sports Camp Registration
*$40 per sport for the first 2 sports; $60 to participate in all 3 Sessions. (Example - Sign up for Volleyball and girls basketball, cost is $80. Sign up for Volleyball, girls basketball and softball, cost is $60. This is an incentive to sign up for all 3 sports!)
*Campers that stay all day are encouraged to bring a sack lunch/snack/drink. No need to pick-up in between, we will take all campers to Eagles Hall to cool down and relax before their next session begins.
*Please complete 1 form per camper.
*All campers will receive a t-shirt.
*Payment can be sent in to ECA through June 10. Checks made out to Elkhart Christian Academy.

I give my permission to the athletic trainers and coaches to treat my child listed above in case of accident or injury. I fully understand that they will, to the best of their ability, assess and treat the injury or sickness. I also understand that by typing my name in the box below, I waive all responsibility of accident, injury or sickness against Elkhart Christian Academy and its employees/coaching staff.
