2020 STEM Shoppe Loan Materials Request Form
Welcome to the STEM Shoppe! Our check out procedures will be changing in 2020 as we upgrade to a new system.
In the meantime, please use this form to request to borrow items from The STEM Shoppe. Loan items are available to educators within a 75 mile radius (approx. 1 hour drive) of Tulsa. You will be contacted via email for confirmation date and delivery details. Items are available for two week periods and delivery/pick ups are generally scheduled on the first and third Mondays of the month. Please note that items may not be available for the date(s) requested - advance planning is recommended.
