Cast Member Behavior Contract
To be reviewed by child/ward WITH parent/guardian.
This document must be signed by each cast member and a parent or guardian and returned prior to the first class attended. Ellis County Children’s Theater’s purpose is to instruct children in music, dance and theater skills which will increase individual and team growth. -------------I agree to make ECCT a safe place. For the benefit of other cast members, the staff, and myself, I agree to conduct myself in the following respectful manner: 1) I will respect myself and others. 2) I will listen to others including staff and fellow cast members. 3) I will control my own behavior and use appropriate language. 4) I will not cause physical or emotional harm or bully another cast member. 5) I will follow the ECCT rules and the ECCT schedule. 6) I will respect the environment, equipment, property and other cast members’ belongings. 7) I will wear appropriate clothing and footwear for all activities at camp including rubber soled shoes (NO BARE FEET!). 8) I will refrain from using electronic equipment during rehearsals, including cell phones and all mobile devise. 9) I will not possess or use alcohol, tobacco, illegal substances, or any paraphernalia. 10) I will not engage in or threaten abuse of any kind. 11) I will put away all electronic devices such as cell phones, iPods, iPads, etc. away during the entire session time. -------------------------------------------------------------------------------By E-Signing this section, I am indicating that I have reviewed this Cast Member Behavior Contract with my parent(s)/guardians and agree to abide by the rules. I understand that if I fail to follow any of these rules, I could be disciplined and/or sent home from camp at the discretion of the ECCT Director. In the event that I am sent home, no refund will be given. Following these rules will help camp be a safer, happier environment for everyone. *

Release & Hold Harmless Agreement
In consideration of my child or ward’s participation in the Ellis County Children’s Theater (ECCT) program, I do hereby, for my child or ward and myself, my heirs, executors, administrators and assigns, release, hold harmless and forever discharge ECCT and their board members, agents and employees, of and from any and all liability and claims, for damages, expenses, personal injury or death, which may arise in the future, related to, connected with, or growing out of participation in said programs, including but not limited to liability and claims arising from the negligence of the parties hereby released. I understand ECCT does not provide any hospitalization or medical insurance to cover my child or ward for hospital or medical expenses incurred, related to participation in the program, and I am solely responsible for the payment of any and all hospital and/or medical bills. ----------------------------------------------CODE OF CONDUCT---------------------------------The following rules are designed to protect the students, counselors, and facilities of Ellis County Children’s Theater program. Unacceptable behavior will result in a worthy consequence: 1)We are guests of Waxahachie High School and should act accordingly. 2)Use of profanity, either verbal or through gestures will not be tolerated. 3)Horseplay (biting, throwing, play fighting, spitting, etc.) will not be tolerated. 4)Students should be respectful of each other’s personal property. 5)Students should be respectful of all instructors and counselors. - A student will receive a verbal warning which will reiterate the child’s behavior and state the consequences if the behavior continues. If the child continues to misbehave, the parent/guardian will be called. *PLEASE NOTE: If the behavior is of a serious nature, the child will be dismissed from the program immediately. No refunds will be given for students dismissed due to disciplinary reasons. I approve of my child/ward’s enrollment in the Ellis County Children’s Theater program. I agree to the Release and Hold Harmless agreement. I will take responsibility for seeing that he/she complies with the stated Code of Conduct. *
