Thank you for filling out this form to help us track the spread of COVID-19 like symptoms in our community! This information will help us gain greater clarity on the types of symptoms experienced by our community and areas of impact. This will also help us align response resources in Pitkin County and evaluate strategies to decrease the spread of illness.
If you are sick with mild symptoms and suspect it is COVID-19, please visit this site to learn how you can protect yourself:
If you have severe symptoms:
- Stay home, except to seek medical care
- Call a primary care physician first (do not just show up)
- Do not show up at testing facilities without a confirmed appointment
- If you are experiencing any life-threatening symptoms, call 9-1-1.
