This form is made available for the purpose of collecting information to help establish the "Parent Portal" web service. This service will give the primary guardian of students enrolled in Cleveland Public Schools' grades 3-12 the ability to monitor their student's academic performance on the internet.
Once the form has been reviewed and the enrollment accepted, guardians will be able to log into a web site and view information such as their students' grades, assignment due dates, if assignments are missing, and attendance.
This service is only meant for guardians listed as the primary contact for their student in our management system. To request a username and password for this web service please fill out the following form as accurately as possible.
While other considerations can be made at a later time, our current goal is to get one guardian for each student to be enrolled in the service and actively use it. After some time has passed we will notify primary guardians by email with a process to add a second user to this web service.
