1. Assumption of Risk - On behalf of me and my party, I voluntarily assume full responsibility for any risks of loss, property damage or personal injury, including death, that may be sustained as a result of my activities.

2. Liability Release - In consideration for DAI allowing our party to access Plaza Blanca and the Dar al Islam property, I and on behalf of my party, agree not to sue and releases DAI (and its officer, board members and associates) from any and all liabilities, claims, demands, actions, causes of actions, costs and expenses of any nature whatsoever arising out of any loss, damage, or injury, including death, that may be sustained by me or to any property belonging to me, arising from activities or while upon the premises where the activities are being conducted.

3. Indemnification - I and on behalf of my party agree to indemnify and hold harmless DAI (and its officers, board members and associates ) from and against any loss, liability, damages or costs, including court costs and attorneys’ fees, that DAI may incur arising from my involvement in activities.

4. Warranty of Physical Fitness - I and on behalf of my party warrant that everyone in the party is physically fit and in a condition that will allow participation in the activities. The party maintains medical insurance that covers accidents and illnesses while participating in activities on DAI property, and assumes full responsibility for payment of medical expenses not covered by the party's insurance incurred as a result participation in activities on DAI property.

5. Photographic Release - I and my party irrevocably agrees to grant DAI the right to publicize any and all photographs, images, videos, and audio in connection with activities on DAI property, including those showing or depicting the undersigned. DAI may use any photographic images, videos, and audio in any reasonable manner DAI deems to advance the goals of DAI with no right of royalties or compensation.
