If you are not the team captain, please, do not fill this form in.

Filling in this form does not confirm your team's participation in the tournament.
In addition to filling the form, you'll have to
- wait for the confirmation from the event organisers
- cover the team's participation fee (following the confirmation)
- register on BCP
