2026 On Line PPA Application
A PDF version of this application will be emailed to you for your records.  All membership fees will be collected at the first tournament of participation - or it can be mailed directly to the PPA using the below address.  Membership fees can be paid by cash or check.  Participation in an event will be allowed with just the completion of the application; however, no prize money will be awarded until the membership is approved.  If you do not include your social security number with the online application, please print a copy of the PDF receipt from your email, add your social security number in pen and mail it to:
