2023-24 TYDE Deactivation Form
Complete this form to deactivate your swimmer's participation with TYDE.  
This form must be completed by the END of the month your swimmer will stop swimming.  June 30th is the last day to choose to deactivate before the end of our season on July 31st.
  • Deactivations will be dated the end of the month your swimmer wishes to stop participating (Example - Swimmer will swim until the end of the month of June - date will be June 30th). 
  • Deactivations will be processed after the first week of the stop month.
  • No deactivations will be processed after the 8th of each month.
  • Your swimmer will be removed from the roster and no further payments will be drafted.

Deactivation Effective Date - Last day of the month you will stop swimming (last date of the month). *
This is the last date the swimmer will practice with the team. It will be the end of a month (a month will not be prorated ). This will notify TYDE staff to remove the swimmer from the TYDE roster starting the first of the next month and stop the next month's draft which takes place on the 15th of next month.

This serves as your electronic signature that you wish to cancel TYDE for your child. Per the Terms and Conditions signed at registration, this form must be submitted by the last day of the month that your swimmer wishes to stop swimming to be able to stop the upcoming draft. You will receive an email confirmation when the deactivation is complete.
A copy of your responses will be emailed to the address you provided.
