Perennial Delights Order Form

This order form is organized based upon the product offerings on my website. Please refer to www.perennialdelights.com to make your selections. If you are looking for something that is not listed in my product offerings, please select "Custom Delights" and enter as many details as possible. 

Requests for dates will be on a first-come, first-serve basis so be sure to enter both your first and second choice of dates based on availability on the calendar on my Ordering information page. When placing orders, please allow a minimum of 1 week to avoid an additional charge for last minute orders. Please be mindful that everything is baked fresh to order and last minute requests may not be feasible.  

Confirmation of order will be sent along with an invoice for payment based upon availability
