Fee of $180 for the Berkeley Math Circle Spring 2017: Deadline is December 6, 2016
The fee of $180 is payable ONLY by check written to United Math Circles Foundation and mailed to Berkeley Math Circle - Upper, c/o Prof. Stankova, Evans Hall 970, Department of Mathematics, University of California at Berkeley, Berkeley, CA 94720-3840. Alternatively, you can bring the check in person and slide it in a sealed and addressed envelope under the door to Evans 713. On the note line of the check write the name of the student and the BMC group you are applying for. The BMC and the Thursday MTRW program are independent programs. Students cannot be switched from one program to the other. No fees can be applied from one program to the other, rolled over to future programs, or used in any other way.

Latest School Report Card and a Recommendation Letter from a Math Teacher: Deadline is December 6, 2016
The latest school report card must show, among other things, grades in mathematics, in science, and in English (reading/writing), as well as behavior grade/description. The letter from the math teacher must address the mathematics knowledge, performance and potential of the students and must be sealed in an addressed, sealed, and signed envelope. Both report card and teacher's letter must be included along with your check for $180 and received by December 6, 2016 in the Math Department of UC Berkeley, as specified in the previous question.
Payment Rules and Conditions
No check will be deposited until a student is accepted into the program. Once a check is deposited, it is neither fully nor partially refundable. A deposited check cannot be exchanged for another student, cannot be rolled over to another program or used for any other purpose, regardless of changed scheduled or other reasons. Bounced checks will incur a $30 fee, which must be paid to United Math Circles Foundation. Please write "I agree" in the answer box to indicate that you understand and accept these conditions.

Notification of Enrollment Decision by end of December 2016. No Email to Staff Members on Non-Emergency Questions at ANY TIME! Consent, Rules of Behavior and Expectations, and Photo Release Forms in January 2017.
We will communicate with the applicant's family only through the specified email(s) on this form. Enrollment decisions will be emailed likely by the end of December 2016 (or earlier). Do NOT inquire about the status of your application until we contact you. The program is running on low resources as we try to keep it affordable for everyone. Thus, we dedicate all of our resources to maintaining the high academic level of the program. We have no resources to answer random emails from the 250 students in BMC-Upper and twice as many parents. Hold off your questions until the spring semester starts (January 17, 2017) and your questions can be answered in person by other parents in the program or by staff members during our office hours. An emergency email is NOT caused by procrastination, disregarding the rules of BMC, or not paying attention to announcements, and is NOT intended for gathering information about the circle. An emergency email is one that requires immediate action to be taken by a BMC staff member. Families who break this rule and use BMC staff or faculty emails for non-emergency questions will seriously jeopardize the acceptance and participation of their children in the program. Additional Consent Form, Rules of Behavior Form, and Photo Release Form will be emailed to newly accepted families in January 2017, to be signed and brought to the first meeting of the circle in January; no child will be allowed to enter the circle in Spring 2017 until we receive these extra forms.
