We will begin reviewing responses on December 16th and continue on an ongoing basis. We will begin to notify interested parties to confirm their acceptance beginning January 6th. We will continue to review responses until attendance is full and will update this form once we've reached attendance limits.
This form consists of three pages of questions. It should take a minimum of 10 minutes to fill out. Some questions are open answer, so we hope that applicants will be able to spend more time (20-30 minutes) to consider their responses. Most questions are optional, but they help us to create a cohort of attendees with complementary experience and skills.
We do not know if this form automatically saves, so we suggest you fill it out in a single sitting.
