Cards for Kids Application
This application is for kids for which Streator Public Library is their home library. Please fill in all boxes. Children under the age of 18 require adult consent. Adults who complete the application and sign with a digital signature are consenting for their minor child to have access to library resources. Adults who sign the application  for their minor will be responsible for any lost, damaged material or any other charges that may occur.  You will be emailed when the card is ready for you to come in and pick up with your child or if there are any questions.
