Nominations Form
River Valleys CoC accepts nominations for CoC leadership and committee membership at any time and reviews nominations on an annual basis.  

This form is meant to collect nominations for all CoC leadership roles: Co-Chairs and CoC committee members. 

Members may nominate themselves or others for any open seat they are eligible to fill. All nominations will be reviewed by the Executive Committee. A slate of nominations will be presented at a regular Full CoC meeting, and a vote for all seats will be held on or before the January Full CoC meeting date.  

This form has three sections:
  1. Information about the person making the nominations (you)
  2. Nominations for CoC Co-Chair
  3. Nominations for CoC Committee membership
