This form is used for requesting manual sales invoice for orders thru UNIQLO app or website.

Please be advised that original copy of Transaction receipt shall be exchanged upon claiming of manual Sales Invoice at your preferred store. In case the original copy of transaction receipt is lost, a notarized affidavit of loss must be provided before Uniqlo can process your request.
