Applicants are responsible for contacting the EPL with proof of identity, residency and annual fee, if applicable. You may visit us at 20 State Street, Ellsworth, ME with documents and payment to receive your card, or you can mail payment and photocopies of proof to our address, and we will mail your card back to you. We look forward to hearing from you. *
