Make-up Service Request Form
Thank you for contacting DivaStyle By Rose, LLC for your customized luxury makeup experience. We ask that you allow 24-48 hours for a response to confirm if the requested date and time is available. If available, a non-refundable/non transferable 50% deposit is required to complete your booking. Once the deposit is received the makeup services request is finalized and your appointment is secured.

It is imperative that you allow a minimum of ONE hour for your requested service(s). My studio is located on the Northwest side of Charlotte, NC. It is located off of Brookshire BLVD & approximately 25 minutes from Uptown Charlotte & 35 minutes from South Charlotte. This is important when selecting your appointment time to allow travel time coupled with the possibility of traffic. Lastly, it is required that you review FAQs & Policies prior to booking.

Date of service requested. Holiday and Sunday fees are an additional $15 fee.
Time of service requested? Appointment time frames will vary depending upon requested services.  Appointments scheduled before 9:00am and after 6:00pm require an additional $35 fee. Please allow a minimum of 2 hours for makeup application and travel time to your event.
Location where you would like your services provided? Please include the complete address (Preferred entrance door/suite number, etc.) Travel and set up fee starts at $45 additional contingent upon requested location. A 4’ to 6’ table is required upon arrival along with an outlet near the table.

Please note travel services are only for soft or full glam services.

Please note if you would like to travel to my studio (located in Northwest Charlotte) please type studio and the address will be provided upon receipt of payment.
