1. Identify a PG faculty or staff member willing to serve as the organization's advisor. Advisors must be fulltime employees of Purdue Global. They will be required to sign the Faculty Advisor Agreement Form.

2. Complete this form and the Student Handbook Acknowledgment Form.

3. You must provide at least six active students in this registration form in order to start the new organization.
