This form will notify the department that you intend to become an organizational profile gatekeeper who is responsible for maintaining an accurate organizational profile in OATH. You will need your OATH username before starting this form. Annual submission of an organizational profile is required for all designated trauma facilities, EMS agencies and air ambulance services. This process is not for the EMS education program gatekeeper role.

An organizational profile must be submitted before your organization can:
• Submit trauma registry data to CDPHE
• Submit EMS patient care reports to CDPHE
• Apply for trauma facility designation
• Apply for grant funding
• Apply for a waiver to EMS scope of practice
• Apply for an air ambulance license
