This form is to request funding for a permanent asset for a student organization, NOT for events.

Please submit this form by Friday at 5:00 pm CT in order to be considered in Senate the following Tuesday. Requests must be submitted for consideration in the Senate meeting at least two (2) full weeks before the event in question.

How will you record the effectiveness of these items? Include a date that you will report on your successes with a measurable value. For example it could be the number of students that have used the item, written accounts by students on how they used the items, pictures documenting the item in use, etc. Basically, what is you're plan to show senate these funds were used effectively. Include specific dates.
