We are excited to welcome you to The Floor Dance Academy!

Trial classes are offered for students who would like to try a class before committing to enrollment.

***THIS FORM IS A REQUEST FOR A TRIAL CLASS.

***YOUR CHILD IS NOT REGISTERED FOR A TRIAL CLASS UNTIL YOU RECEIVE AN EMAIL CONFIRMATION DIRECTLY FROM info@thefloordanceacademy.com AND PAYMENT HAS BEEN RECEIVED.

TRIAL CLASS POLICIES & GUIDELINES:

• All trial classes must be requested by filling out this form a minimum of 72 hours prior to the class. We will not accommodate students without a form submitted.

• Multiple classes can be requested on each form, but please fill out ONE form per child.

• ONLY CHOOSE THE CLASS/ES YOU WOULD LIKE YOUR CHILD TO TRIAL. ALL CLASSES CHECKED OFF WILL BE CHARGED FOR.

• Trial Class Rate : $35/class (not eligible for discounts)

• A confirmation will not be sent until tuition has been paid.

• REGISTRATION IS NOT COMPLETE UNTIL YOU RECEIVE A CONFIRMATION FROM US. Do not arrive to a class unless you have received confirmation.

• Available trial classes are listed below by the day of the week.

• Some classes require instructor approval. We will notify you of your approval before the class. Please review the class schedule on our website to review the classes that require studio approval to enroll.

• AFTER THE TRIAL CLASS, if you wish to enroll, EMAIL Dorie at info@thefloordanceacademy.com. A verbal request to the teacher or on-site desk manager does not constitute enrollment. Class Registration is ONLY CONFIRMED & ACCEPTED via email to Dorie. A spot will be held in the class for 48 hrs after a student participates in a trial class. If a student does not register within 48 hrs, their spot will be offered to the next interested student.

• Existing students who wish to try a class that they are not currently enrolled in must also pre-register by filling out this form.

• Not all classes on the schedule can accommodate trial students if the class is already at capacity. We will do our best to find a class for you if your requested class is unavailable.

• Trial classes will only be scheduled during the below weeks:
