Basketball Clinic is on Tuesdays from January 9 until March 12
5:30 - 6:30 - 2nd graders
6:30 - 7:30 - 3rd graders

In order to participate in the clinic, please complete the following - 

1. Completed Registration Form (current page)
2. Completed Permission and Insurance Form (Page 2)
3. Photo Release (Page 3)
4.Copy of Insurance Card (Front & Back) - email to basketballclinic@demaresthsa.org
Please note: Children will not be able to participate without insurance
5. $25 - Check, Cash or online payment - Check made payable to Demarest HSA - in an envelope labeled "2/3 Grade Basketball Clinic", with your child's name and their teacher. 

This can be sent into school with your child. 
($25 returned check fee)

Please arrive promptly to pick up your child. Practices are for parent volunteers and students registered to play, only.
