To ensure intent for re-enrollment of your child, this form must be completed by April 30, 2026. If you have any difficulty completing this form online, please contact the office at 541-774-3900 for assistance. If we do not hear from you by April 30, 2026, we understand that your child(ren) WILL NOT be attending KUA for the 2026-2027 school year, and we will offer the space(s) to new applicants. / Por favor complete el siguiente formulario para volver a inscribir a sus hijos para el año escolar 2026-2027 en KUA.
