If a parent wishes to be notified any time his/her student was in the classroom with a student who has recently tested positive for COVID-19, please fill out this form.  The district will then provide notifications to you when there is a student in class that tests positive, but the specific identity of the student will not be released.  We will also not be able to inform you of the proximity that your student was with the positive case.

A separate form must be completed for each of your students for which you wish to receive these notifications.
