Your registration will be secured once we:

1. RECEIVE PAYMENT either online (look for the link once you submit the form and click the correct amount) or with a check payable to: "SUSTAINABLE SOUTHBURY"  sent to Sustainable Southbury, 1202 Purchase Brook Road, Southbury CT 06488.

2. RECEIVE YOUR PROOF OF INSURANCE FORM.  Please email the form to info@sustainablesouthbury.org (preferred) or send to the address above.  See the insurance requirements and a sample proof of insurance document in our TERMS AND CONDITIONS available at the link below or on the website.

THE INSURANCE MUST COVER BOTH THE TOWN OF SOUTHBURY AND SUSTAINABLE SOUTHBURY.  Most vendors submit two forms as shown below.
