Thank you for your interest in conducting research in Chesterfield County Public Schools (CCPS). We are committed to supporting high-quality research which advances educational practice. Data collection and research efforts within CCPS require approval.

Please reference the "Office of Research and Evaluation Research Guidelines and Application" for more detailed information about the process and a list of all required information prior to filling in this form.  

A copy of your responses will be sent to the email address provided. This email does not need to be the same as that provided for the primary researcher.
