1. Read and complete the entire application, and select "SUBMIT" at the end of the application. You must include a valid email address, which you understand must be checked daily for ELC communication and requests for your response.

2. Ensure that a Teacher or School Representative provides a recommendation for you and submits the recommendation online by the due date. With the submittal of your application, a message will be automatically sent to them asking them to access, complete, and then submit the online Teacher/School Representative Recommendation Form via the link available on the ELC Program website noted above. Only one Recommendation is required; additional ones will not be used.

3. Attend one of the two Orientation Sessions with your parent or guardian.

4. At one of the required Orientation Sessions, pickup, review, sign, and submit the Rotary Participation Agreement and Release Form with the required signatures.

All FOUR of these steps are required for an Applicant to be considered for participation.

Who is Eligible?
• Applicants enrolled in an educational system under the laws of the State of California.
• High School student who arein their Junior year from selected high schools in the communities supported by this ELC Program.
What Does the Conference Include?
• Dynamic keynote address with a motivational message from a well-known local business leader.
• Conference students grouped into teams, with each intended to have 3 girls and 3 boys, from 6 different high schools. Each team has 1 - 2 adult mentors, who are Rotarian volunteer “coaches/mentors” called Rotary Team Advisers (RTAs).
• “Hands-on” team activities to build collaboration and business skills.
• Focused, interactive, short presentations by successful business leaders covering all key aspects of forming and managing a business: finance, marketing, talent management/human resources, presentation skills, ethics, etc.
• Team presentation of a competitive business plan, while demonstrating creative and collaborative skills.
• Meals, snacks, and water will be provided. Students’ personal expenses, such as for extra snacks or drinks from vending machines, are the responsibility of the student.
Who Should Attend?
Students who:
• Are interested in learning about business.
• Want to start their own business later in life.
• Like to meet new people.
• Enjoy working on a team with people and coming up with cool new ideas for business products and services.
• Are willing to invest three days in themselves and their knowledge base that could help them in the future.
• Are open to learning about entrepreneurship and what it takes to be an entrepreneur.
• Are somewhat uncertain about what they want to be or do in the future.
• Are motivated to grow in any of the above areas.

There is NO requirement for a certain GPA or for set of completed courses.

Expectations for Accepted and Participating Students
Students, who are accepted, must immediately reply to the confirmation email they receive. If they cannot attend, they should immediately reply or call the ELC Hotline to release the space for an alternate student.

Accepted ELC students and their Parent or Guardian MUST ATTEND one of the two Conference Orientation Sessions held after work in the early evening. This is your opportunity to learn more about the conference and to meet the Rotary Team Advisors or RTAs, other accepted students, and Conference Committee leaders, as well as have questions answered. At the session, you will also receive, review, sign, and hand in two forms: (1) the Rotary District 5170 Youth Services Event Permission Slip & Waiver Form and (2) your ELC Student Participation Agreement. Your attendance and these two signed forms, along with the submittal of a Recommendation for you, WILL COMPLETE YOUR APPLICATION PROCESS.

Each applicant will be expected to complete two online surveys, one prior to and one following the event, which should take the less than 10 minutes each. The first survey will be used to gather anonymous information from the students that will be shared with the students as part of the curricula and the second survey, to collect anonymous information to assess the results of the program, as well as feedback to adjust the curricula.

The applicants should be willing to share their experience in a short presentation at a meeting of one of the sponsoring Rotary Clubs, if asked.

Selected conference students are expected to arrive at the drop-off location on time and attend all scheduled sessions of the Conference with their team. Being on time and prepared to participate with peers is a sign of respect for your team, the Conference and the RTAs. At the end of the Conference, students must be picked up at the pick-up location by time noted.

Once signed into the Conference, a student may not leave the Conference early. In the case of an emergency or special situation, a signed approval is required from one of the Conference Co-Chairs and the student must be picked up by a parent/guardian with appropriate identification.

All Conference students, once signed into the Conference, must use Rotary-arranged transportation to and from the venue. Transportation is provided by contracted bus. Under no circumstance are personal vehicles of students or friends for driving allowed at the Conference.

Information regarding physical limitations requiring special accommodations or special medical dietary must be included in the application and in the Rotary District 5170 Youth Services Event Permission Slip & Waiver Form to enable Rotary personnel to inquire into the necessary accommodations prior to arriving at the venue. Vegetarian options are available for all meals.

Any medication, whether prescribed or “over-the-counter/retail”, must be fully identified on its original container. This information must be included in the application and in the Rotary District 5170 Youth Services Event Permission Slip & Waiver Form. In addition, the student must inform the RTA of the need to take medicine. Students may not have in their possession or in their rooms any unapproved medicine or alcohol/wine/beer or other controlled items, or weapon. A student confronted by another student with any of these items or any other dangerous practice is required to seek out adult help (usually their RTA) in distancing him/herself from the situation.

PARTICIPATION AND CONDUCT - The end of each Conference day may vary, but “lights out” and sleep time are strictly supervised and enforced. Any Conference participant disturbing the group or other venue guests (the public) is subject to disciplinary action, which could include having the parent or guardian pick them up immediately. Conference participants are to conduct themselves at the Conference so as to bring credit to themselves, their school, their parents and the Rotarians. Violation of these rules may result in termination of participation in Conference activities at any time, day or night.

- Personal toiletries, necessary medications, hair dryers and other grooming products.
- Casual, comfortable, and appropriate clothing for both the working and team-building sessions at the Conference. (Please keep in mind that there can be cool winds and fog, therefore warm outerwear may be necessary.)

Rotary District 5170 Youth Services Event Permission Slip & Waiver Form and Student Participation Agreement
The Applicant is required to attend, with at least one Parent/Guardian, one of the two Orientation Sessions, at which two forms will be passed out for review and signature. One form, the Rotary District 5170 Youth Services Event Permission Slip & Waiver Form, will be pre-filled out with the information you have provided in this Application and will require the Parents(s)/Guardian(s) signature. The second form, shown below, covers the expectations for participants and requires the participant's signature.
ELC Student Participation Agreement
The following is the information that each student will be required to read and sign at one of the
mandatory Orientation Sessions. (THIS FORM IS NOT TO BE SIGNED ONLINE.) This Agreement pertains to the 2018 Enterprise Leadership Conference.

With my signature, I hereby:

1. Confirm that I have received, read and understood the contents of the Rotary Expectations of Accepted Students and application information.
2. Confirm that the information provided in this application is accurate at the time of my signature.
3. Confirm that I understand the nature of the ELC experience and the requirements for the students’ participation.
4. Confirm that the student is available to participate on all of the stated dates for the Enterprise Leadership Conference.
5. Confirm that the student is now a high school junior in credits and expects to be a high school senior in the fall following the event.
6. Grant to The Rotary Clubs permission to copyright, use, reuse, publish and republish any photograph/video taken of the student by The Rotary Clubs or those commissioned by The Rotary Clubs. This right applies to any reproductions in any form, used alone, in a composite or with any printed matter or advertising copy, for any purposes of trade, advertising, publicity, promotion or education, without restrictions or limitations.
