Eric Moss Fitness - Amazing 12 Application Form
The Amazing 12 is considered to be the top body transformation program because of the massive results it produces in a very short period of time (an intense 12-week training and 3 phase supportive nutrition cycle that is done 5 days per week approximately an hour each session for 12 consecutive weeks with additional cardio 5 days per week). In order to keep consistent with the high standard of success that the program promises, all the requirements must be met. This includes both the training and nutrition components. If you can’t commit to each of the components consistently for the duration of the 12-week cycle, the Amazing 12 Body Transformation Program may not be right for you at this time.

If you answered "yes" to the previous question, how do you plan to work around this to ensure the successful completion of the program?
The 3 phase nutrition protocol is designed to have a specific outcome (simultaneously gaining muscle while burning fat). Even though the protocol can be flexible in its implementation, there are very specific rules of engagement that must be met consistently. To ensure success, both a food journal and a food journaling app are used, and preparing your meals in advance is recommended. Is there any reason why this could possibly be an issue? *
Part of the training component of the Amazing 12 program includes additional cardiovascular training (running and sprints) which varies between 3 and 20 minutes in duration and is done 5 days per week for the duration of the 12-week cycle. This would be done on your own, outside of the studio. A running app that can calculate distance is recommended. Is there any reason you can see why this might be an issue? *
If you answered "yes" to the previous question, how do you plan to work around this to ensure the successful completion of the program?
The Amazing 12 requires a high level of work ethic and commitment. You must do each of the workouts and the nutrition rules of engagement to the best of your ability each day of the cycle. Are you willing to give the effort required each session to get the optimal result that the program delivers? *
Participation in the Amazing 12 Body Transformation program involves documenting our journey together through pictures and videos. These pictures and videos are there to show both the success of the program to yourself and may also be used for marketing purposes and future promotion of the program. The “after” pictures will be taken by a professional photographer and may be displayed publicly on both amazing12.com and EricMossFitness.com. Are there any concerns or issues with this? *

On a scale from 1 - 10 what is your level of commitment to following the guidelines of the Amazing 12 program for the duration of the 12-week cycle? *
I'm not committed at all.
I'll do whatever it takes.
