Participation in both this screening questionnaire and the research interview is completely voluntary and you can choose to withdraw at any time. All identifying information collected will be kept strictly confidential.

If you are selected to participate in this interview:

• We will be in touch with next steps using the contact information you provided.

• Consent will be obtained before each interview to ensure participants are fully informed and have had the chance to ask any questions they may have. The interview will be recorded for data analysis purposes.

• Data gathered in this questionnaire and the interview will be de-identified prior to data analysis.

If you are not selected to participate in this interview:

• If you choose, we may keep your information in our system for up to six (6) months to alert you of future paid research opportunities that you may qualify for. This is entirely optional.

• If you choose to withdraw your response after submitting this form or if not selected for an interview, your data will be deleted immediately.
