Waiver and Release of Liability
I, the undersigned, being the parent or legal guardian of the above-named Participant (if under 18), or the Participant myself (if 18 or older), acknowledge that participation in the STEM Summer Camp involves activities that include 3D printing, heated build plates, and computer use.

I understand that these activities carry potential risks, including but not limited to minor burns, electrical shocks, and other injuries. I acknowledge that all necessary precautions and safety measures will be taken by the event organizers to ensure the safety of all participants.

In consideration of the Participant’s participation in the event, I hereby release, waive, and discharge Metro Homeschool Robotics Inc., its officers, employees, volunteers, and agents from any and all liability, claims, demands, actions, and causes of action arising out of or related to any loss, damage, or injury, including death, that may be sustained by the Participant, or to any property belonging to the Participant, while participating in the event or while on the premises where the event is conducted.

I further agree to indemnify and hold harmless Metro Homeschool Robotics Inc., its officers, employees, volunteers, and agents from any loss, liability, damage, or costs, including court costs and attorneys' fees, that they may incur due to the Participant's participation in the event.
Medical Treatment Authorization
In the event of an emergency, I authorize Metro Homeschool Robotics Inc. to secure from any licensed hospital, physician, and/or medical personnel any treatment deemed necessary for the Participant's immediate care. I agree that I will be responsible for payment of any and all medical services rendered.
Photography/Video Release
I grant permission for Metro Homeschool Robotics Inc. to photograph and/or videotape the Participant during the event and to use these images in promotional materials, social media, and the organization’s website.
Acknowledgment of Understanding *
I have read this Waiver and Release of Liability, fully understand its terms, and understand that I am giving up substantial rights, including the right to sue. I acknowledge that I am signing this agreement freely and voluntarily and intend my signature to be a complete and unconditional release of all liability to the greatest extent allowed by law.
