* The purpose of this form is to alert the facilities manager in advance of events and to not overlap or double book meeting areas. Please fill out this form anytime if you will be using the school facilities for a meeting or event that involves more than just yourself and it will start before 8:15 or go past 3:45 PM during the week or anytime on the weekends. Thank you for your cooperation. This form needs to be filled out 3 business days PRIOR to the event or will be subject to cancellation.
