Please fill out the form below if you are interested in donating blood during the LifeShare Blood Drive. You must be 16 years or older to participate. If you are 16, you must stop by Mrs. Frachiseur's classroom to pick up a consent form. The form must be complete with both your and your parent's signature. I also encourage preparing ahead of time to give blood by getting a good night's sleep, drinking plenty of water, and eating a balanced meal. If you have any questions, please see Mrs. Frachiseur.
