HELUS STUDENT TECHNOLOGY DEVICE CHECKOUT REQUEST AND AGREEMENT
Hughes Elizabeth Lakes Union School District (HELUS) may provide Chromebooks or iPad to students enrolled in the District. The District understands the need for students to have access to proper learning materials. HELUS loans its students’ Chromebooks or iPad with no deposit required and expects these Chromebooks or iPad to be returned to the District in good condition.

Chromebooks or iPad that are lost, stolen, damaged, or defaced will result in a demand for payment (see table below). During the time that outstanding fees remain uncollected, and after due process has been afforded, grades, transcript, report cards, and other school privileges may be withheld. If payment becomes necessary, and full payment cannot be made at once, a payment plan will be offered by HELUS. The student will not be allowed to borrow school equipment until restitution for the first device has been satisfied. (Education Code §48904)

This policy has been implemented to ensure that the public resources provided to the District are used efficiently and effectively and to ensure that all students have the instructional materials they need and deserve.

If you have any questions concerning Chromebooks or iPad issues, please contact your school within five
(5) days of receipt of this notice. Chromebooks or iPad are due when changing classes, checking out of
school, or at the end of the school year.

I will never leave my device unattended in an unsecured or unsupervised location.
I will never loan out my device to other individuals.
I will know where my device is at all times.
I will charge my device's battery to full capacity each night.
I will keep food and beverages away from my device since they may cause damage to the device.
I will not disassemble any part of my device or attempt any repairs.
I will protect my device by always carrying it in a secure manner to avoid damage.
I will not leave my device in a vehicle as the heat and cold can damage and/or ruin the device.
I will use my device in ways that are appropriate for education.
I will not place decorations (stickers, markers, writing, etc.) on the device.
I understand that the device I am issued is subject to inspection at any time without notice and remains the -property of the Hughes Elizabeth Lakes Union Elementary School District.
I will follow the policies outlined in the Technology Policy Handbook and the District Acceptable Use Policy while in school as well as outside the school day.
I will file a police report in case of theft or damage caused by fire.
I will be responsible for all damage or loss caused by neglect or abuse.
I agree to pay the full replacement cost of my device, power cord/charger, in the event that any of these items are lost or intentionally damaged.
I agree to return the device, power cord/charger in good working condition at the end of each school year, or if I move.
