The Naval Academy Athletic Association requires us to collect this information.
Allergies to common foods requiring special dietary considerations cannot be supported by King Hall and is a disqualifying condition for attending camp. If your camper has a food allergy or other special dietary need (vegetarian, gluten-free, etc.), you must contact our camp directors for approval before your application can be confirmed. Please do NOT proceed with payment until you receive approval as we are unable to refund credit card fees. If no allergies, please enter "none."

I/We being the legal guardians of the above applicant authorize the Naval Academy Athletic Association, the specific camp and its agents, permission to request medical treatment as necessary to insure the well being of the applicant.
