HCCN & Navigate Assistance Request Form
MATC staff are welcome to submit this form anytime for Holistic Coordinated Care Network (HCCN), Navigate, and other related navigation issues, permission requests, training requests, system errors and process improvement suggestions. 
 
With most EAB requests, specific examples or student ID numbers are needed to troubleshoot.  An example would be a screenshot error message or provide specific student and/or example.
