Videotaping & still photographs will be taken during this event. By typing my name below, this authorization constitutes permission for my child’s participation during Bible Camp & in the videotape and/or still photographs, which may be used for future promotional efforts, including the St. Luke website and St. Luke Youth & CCE Facebook pages (no volunteer personal pages or social media are included in this authorization).

For those registering on or before June 25th: Once we have received this form, we will email you separately to confirm your child's registration AFTER receiving payment in the CCE office or via the online Diocesan payment system. 

To make a payment online (**note there is a $3 fee per transaction):

3) Under "choose your organization" select: St Luke School - Brownsville
4) Complete form as needed - $25 per child (additional $3 transaction fee is charged at the end)
5) Submit your payment info. You will receive a confirmation email - with a confirmation number for the payment. Please forward this receipt email to: cce@stlukecc.org with your child(ren)s names included in the subject line.
