Minshew - Parent or Guardian Consent Form: Student Participation in Clubs & Organizations

In accordance with recent legislation, written parent/guardian consent is now required for students to participate in any student club or organization, whether curricular (e.g., academic teams) or non-curricular (e.g., interest-based).

This form serves as a general consent for your child to participate in any approved club or organization offered at their campus during the 2025–2026 school year.

  • By signing this general form, you are permitting your child to participate in any club or organization they choose to join.

  • If you prefer to grant permission for specific clubs only, you may request and complete a club-specific consent form from the campus club sponsor.

  • No student may participate in any club or organization without written parent/guardian consent on file

NOTE: If you have multiple students on one campus, fill out this form for each of your children.
