In accordance with recent legislation, written parent/guardian consent is now required for students to participate in any student club or organization, whether curricular (e.g., academic teams) or non-curricular (e.g., interest-based).
This form serves as a general consent for your child to participate in any approved club or organization offered at their campus during the 2025–2026 school year.
By signing this general form, you are permitting your child to participate in any club or organization they choose to join.
If you prefer to grant permission for specific clubs only, you may request and complete a club-specific consent form from the campus club sponsor.
No student may participate in any club or organization without written parent/guardian consent on file
