Please complete this form thoroughly for each child participating in 2026-2027 Faith Formation Program, grades K-12, at Holy Family Parish.

This year's Family Orientation will be held on Sunday, September  20, 2026 at 9:00 a.m., in the parish hall. Please contact the parish office with any questions you may have.

Religious Education /Faith Formation Cost:
$35.00 per student $70.00 total for family of 2 or more,  $50.00 per student for confirmation student, which includes fee for confirmation retreat. Cash is accepted and checks can be made payable to Holy Family Parish and submitted to the office by Sunday, September 20, 2026.
