The following pricing is for Early Bird registrations and deposits received on or before April 30, 2019. Early Birds have several benefits:

1) Enjoy a reduced booth rate and get 2018 rates for the 2019 festival,
2) A higher likelihood of receiving preferred booth locations, and
3) Extra publicity because your business and web site will be listed on our Exhibitor page as visitors browse the Collinsville Hot website in the months leading up to the festival.

Booth Deposit and Refund Policy *
I understand that if I cancel on or before April 30, 2019, I will receive a 100% refund of booth fees paid. If I cancel after April 30, 2019 but on or before June 20, 2019, I will receive a 50% refund of booth fees paid. I understand that no refunds will be provided if I cancel after June 20, 2019.

I understand that my application is not considered received until I complete payment of the booth fee and that delayed payment will impact getting my preferred booth locations. -------- Note: All payments are to be made through PayPal at www.CollinsvilleHot.com/artisan_registration. When making payment through PayPal, it is not necessary to have or create a PayPal account. You can simply enter your credit or debit card for the payment.

Waiver of Liability *
I agree that property brought to this show is at my own risk. I also agree that setting up for, exhibiting at this event, and removing my property at the end of the event is at my own risk. I understand that I am at risk of loss or damage to my property, and risk of injury to myself or my representatives. I assume the risk of loss or injury while exhibiting at this event and agree not to hold the Town of Canton, CT or Canton Main Street, Inc and either organizations officers, agents, sponsors or event volunteers liable for any injuries or loss that I may suffer while participating in Collinsville Hot 2019.

Connecticut Sales Tax *
I understand that a CONNECTICUT SALES TAX NUMBER is required for all exhibitors and that I am responsible for collecting all sales and use tax as required by law. I agree that I will remit payment of required taxes to the Connecticut Department of Revenue Services using the appropriate forms as required by law.

I understand that a Farmington Valley Health District (FVHD) permit is required if I will offer food items for consumption or sampling at my booth.
