Vendor Application Form
Welcome to the Glass Smoke Boutique Vendor Application Form!
Thank you first and foremost for your support and desire to vend with us!

We do not request any vendor fees outside of the $5 fee for renting equipment from us, if needed. (ex. $5 per canopy or $5 per table).

Each Vendor is responsible for operating their business legally and compliantly. 
We are not responsible for how you handle your business. You are responsible for handling your own transactions.

Set Up Time: Vendors can begin setup 30 minutes or more prior to beginning of the event. Break down must be completed by 7:00pm.

We understand that emergencies happen, but no call/no shows will not be permitted and may result in being rejected from further vending opportunities.

You will receive a response from a Glass Smoke Boutique representative within 5 business days of your submission.

Liability Waiver:

By participating in our event as a vendor, you acknowledge and agree to the following:

1. I understand and agree that Glass Smoke Boutique (The Looking Glass LLC) is not responsible for any loss, damage, or injury to myself, my employees, or my property while participating in the event.

2. I hereby release, waive, discharge, and covenant not to sue Glass Smoke Boutique (The Looking Glass LLC) for any and all liability to myself, my employees, or my property arising directly or indirectly from participation in the event.

This Waiver is binding for the 2024 year.
