Transportation Form

This form is to be completed by the parent/guardian of any West Geauga student-athlete who will not return to the school on team-provided transportation following an athletic contest. This includes student-athletes who will be transported by a parent/guardian, another approved adult, or who have permission to drive themselves from the event.

To ensure the safety and accountability of all student-athletes, this form must be completed, signed, and submitted to the coach or athletic department prior to the event. Approval is subject to West Geauga Local Schools Board of Education policy and the discretion of the coaching staff and Athletic Department.

By signing below, I acknowledge and agree that, upon the conclusion of the above-referenced off-site practice OR game, my student-athlete will either:

Be transported from the practice or game location by a parent/guardian or other approved adult; orDrive themselves from the practice  or game location, provided they are authorized by West Geauga Local Schools to do so.

I understand that my student-athlete will not return to the school with the team or school-provided transportation. I further understand that once my student-athlete leaves the supervision of the coaching staff at the conclusion of practice, I assume full responsibility for their transportation and well-being.

I agree to release, indemnify, and hold harmless the West Geauga Local Schools, its Board of Education, employees, coaches, and volunteers from any and all claims, liabilities, losses, damages, or expenses arising from or related to my student-athlete's transportation or activities after departing the practice site via approved alternate transportation or by driving themselves
